Prognosis of Patients With Symptomatic Vertebral or Basilar Artery Stenosis

چکیده
Background and Purpose—There are limited data on the prognosis of patients with angiographically proved symptomatic stenosis of the intracranial vertebral artery or basilar artery. Methods—We studied 68 patients with 50% to 99% stenosis of one of the following arteries: intracranial vertebral (n531), basilar (n528), posterior cerebral (PCA) (n56), or posterior inferior cerebellar (PICA) (n53). All patients had previous transient ischemic attack or stroke in the territory of the stenotic artery and were treated with warfarin (n542) or aspirin (n526). Follow-up was by chart review and personal or telephone interview. Results—During a median follow-up of 13.8 months, 15 patients (22%) had an ischemic stroke (4 fatal), 3 patients (4.5%) had a fatal myocardial infarction (MI) or sudden death, and 6 patients (9%) had a nonfatal MI. Stroke rates in any vascular territory (per 100 patient-years of follow-up) were 15.0 in patients with basilar artery stenosis, 13.7 in patients with vertebral artery stenosis, and 6.0 in patients with PCA or PICA stenosis. Stroke rates in the same territory as the stenotic artery (per 100 patient-years of follow-up) were 10.7 in patients with basilar artery stenosis, 7.8 in patients with vertebral artery stenosis, and 6.0 in patients with PCA or PICA stenosis. Conclusions—Patients with symptomatic intracranial vertebral artery or basilar stenosis are at high risk of stroke, MI, or sudden death. Further studies are needed to clarify optimal therapy for these patients. (Stroke. 1998;29:1389-1392.)

متن کاملCombined external counterpulsation and endovascular stenting treatment for symptomatic vertebrobasilar artery stenosis: two case reports
Symptomatic vertebrobasilar artery (VBA) stenosis has a poor prognosis. Intravascular stents provide a new therapeutic approach, but the long-term outcome of stenting compared with medical outcome is controversial. External counterpulsation (ECP) is a noninvasive method to improve perfusion of vital organs. We report two cases of this combination with ECP treatment in addition to receiving endo...
متن کاملAngioplasty and Stenting of Basilar Artery: Short-term Outcomes
Symptomatic basilar artery stenosis has a poor prognosis. Surgical bypasses are technically demanding and of no proven benefit. A new generation of intravascular stents that are flexible enough to navigate the tortuosities of the vertebral artery may provide a new therapeutic approach. Our two cases, 57 and 52 year-old men experienced a vertebrobasilar ischemia with repeated vertigo and falls. ...
متن کاملTransluminal stent-assisted angioplasty of the intracranial vertebrobasilar system for medically refractory, posterior circulation ischemia: early results.
OBJECTIVE: Symptomatic vertebrobasilar artery stenosis portends a poor prognosis, even with medical therapy. Surgical intervention is associated with considerable morbidity, and percutaneous angioplasty alone has demonstrated mixed results, with significant complications. Recent advances in stent technology have allowed for a novel treatment of symptomatic, medically refractory, vertebrobasilar...
متن کاملRecanalization of symptomatic vertebral ostial occlusion in patients with acute or subacute stroke.
SUMMARY Vertebral artery recanalization in symptomatic stenosis/occlusion remains controversial, as no definite evidence exists regarding this topic. There are only a few reports regarding the feasibility and safety of recanalization in the first segment of the vertebral artery with atherosclerotic vertebral ostial occlusion. We report our experience treating first segment occlusion in 8 patien...
